Welcome members! Thank you for choosing an Individual and Family Plan from Jefferson Health Plans! If your plan has a January 1, 2025 effective date, your initial payment is due by January 31, 2025. For February 1, 2025 effective dates, your initial payment is due by February 28, 2025.

Did you receive a letter about changes to our provider network? You can select a new health care provider near you by using our provider directory at Find a Doctor - Jefferson Health Plans or calling our Member Services Department at 1-833-422-4690 (TTY 1-877-454-8477). We understand that changing providers can be a difficult situation for our members, and we want to help you as much as possible with this transition, if your provider is no longer participating or if you’d like to change providers.

 

On February 21, 2024, Change Healthcare experienced a cybersecurity incident after a ransomware group gained access to the network. Change Healthcare’s systems are still down.
