Engaging Patients: Health Outcomes Survey (HOS) Measures

Every year, the Centers for Medicare & Medicaid Services (CMS) administers the Health Outcomes Survey (HOS), which assesses the ability of an organization to maintain or improve the current physical and mental health status of its members.

A random sample of Jefferson Health Plans’ Medicare members receive the HOS survey.

HOS performance contributes to our Medicare Advantage Star Rating. These ratings matter because they affect plan reputation, member growth, and quality bonus payments.

Because many HOS topics are shaped by what happens in the physician office, provider engagement can make a real difference.

We are asking our provider partners to help reinforce conversations that support member health in the areas reflected in HOS, and of course, help address issues that matter to seniors’ independence, safety, and quality of life.

We value your partnership in making these topics part of routine patient conversations, especially during annual wellness visits, primary care follow-up, and chronic care visits.

Key clinical conversation areas:

  1. Improving or Maintaining Physical Health

    Plan ahead before appointments. Dedicate time to review a patient’s health history and have any lab work results available. This helps to make the patient a part of the decision-making process and more likely to follow recommendations. Ask patients about getting a flu shot and other preventative measures.

  2. Improving or Maintaining Mental Health

    Include emotional health in routine care by asking about mood, loneliness, stress, and whether the member is still able to enjoy daily life.

  3. Monitoring Physical Activity

    Ask members about their ability to do everyday activities and whether they have noticed any decline in mobility, stamina, balance, or strength. Encourage realistic physical activity and functional goals, especially for older adults managing multiple chronic conditions.

    All Jefferson Health Plans Medicare Advantage members have access to the SilverSneakers® fitness center network. Members can also choose to join the Kroc Center, in Philadelphia.

  4. Improving Bladder Control

    Normalize conversations about bladder control or urinary incontinence, since many members will not bring it up unless asked. If you determine a patient would benefit from seeing a urologist, refer your patient (referrals are not required).

  5. Reducing Risk of Falling

    Screen for falls, fear of falling, gait instability, and home safety concerns. Share tips with patients about how they can prevent falls, such as standing up slowly, installing handrails in the shower, and monitoring side effects of medicine.
