Wellness Rewards - Medicare Advantage

The Wellness Rewards Program incentivizes Medicare Advantage members to complete specific health-related activities to earn money on a reloadable card.  

Please refer to the list below for all eligible health activities. Some of the preventive health activities included in the program are tied to current measures included in our QCP program. Leveraging the rewards program and encouraging your patients to complete these activities can help you improve your performance on these QCP measures. Please refer to the 2026 Medicare Wellness Rewards Program Notice in the provider portal for additional details and eligibility requirement on all rewards. CareForward Rewards - Individual and Family Plans

CareForward Rewards for Members

Eligible Jefferson Health Plans Individual and Family Plans members can earn a $25 reward for completing an HbA1c (blood sugar) test and a $25 reward for completing a blood pressure screening between June 1 and December 31, 2026.

Rewards can be used to purchase healthy foods and over-the-counter items at participating retailers and online through the NationsBenefits member portal. Rewards cannot be redeemed for cash or used for gambling, alcohol, tobacco, or firearms. Rewards expire 90 days after activation or on December 31, 2026, whichever comes first.

Helping Members Earn Rewards

Encourage eligible members to complete eligible services to earn rewards while supporting better health outcomes. Providers can identify eligible members through Care Gap Reports available in the Provider Portal Document Manager.

Closing care gaps not only helps members earn rewards but also supports quality improvement efforts and performance goals under the Individual and Family Plans PCP Provider Incentive Program.
