Providers should check the DHS PROMISe system on a routine basis to confirm demographic data, including all service locations/revalidation dates to ensure information is current and have an active Medical Assistance ID. Please visit the DHS webpage at www.dhs.pa.gov/provider/promise/enrollmentinformation/S_001994 for requirements and step-by-step instructions.

Participating provider groups should submit data changes on company letterhead to datavalidation@jeffersonhealthplans.com.

Be on the lookout for your quarterly Provider Data Validation Form from Press Ganey. To maintain directory accuracy, be sure to review and respond as directed.
