Help Prevent Type 2 Diabetes: Refer Eligible Patients to the Diabetes Prevention Program

Type 2 diabetes is a serious but often preventable condition. Jefferson Health Plans offers the Diabetes Prevention Program (DPP), an evidence-based, year-long lifestyle change program designed to help eligible members with prediabetes reduce their risk of developing type 2 diabetes through healthier eating, increased physical activity, and sustainable weight loss. 

To improve access and flexibility, the DPP is available through virtual, distance-learning, and on-demand online options.

How Providers Can Help

Your recommendation can make a meaningful difference. We encourage providers to:

  • Identify patients with prediabetes who may be eligible for the DPP.
  • Discuss the benefits of participation.
  • Refer eligible patients to a DPP provider.
