Effective December 1^st, 2025, Imagenet will be responsible for the intake of paper claims submissions for all Jefferson Health Plans and Health Partners Plans lines of business. As a result, the mailing addresses for all paper claims submissions have been updated.

Please note, there will be no changes to the Electronic Payor ID numbers, and we continue to strongly recommend electronic claims submissions.
