We are pleased to introduce the Jefferson Health Plans Individual and Family Plans Pay-for-Performance Provider Incentive Program.

This program offers eligible primary care providers the opportunity to earn incentive payments based on performance on two HEDIS measures: Controlling Blood Pressure (CBP) and Glycemic Status Assessment for Patients with Diabetes (GSD).

Providers may earn per-member-per-year (PMPY) payments by meeting Tier 1 or Tier 2 benchmarks aligned with the Centers for Medicare & Medicaid Services Affordable Care Act percentiles. Performance will be measured for the 2026 calendar year (January 1 – December 31), with incentive payments issued by March 31, 2027.
