We will continue to offer the member rewards program, Wellness Rewards, to all our Jefferson Health Plans Medicare Advantage members in 2026. Wellness Rewards incentivizes our members to complete specific health-related activities to earn rewards dollars on a reloadable flexible spending card.

Visit www.HPPlans.com/rewards for a full list of eligible health activities. Some of these preventive health activities are tied to current Quality Care Plus (QCP) measures. We have found that encouraging patients to complete these activities may help improve performance on these QCP measures.

Wellness Rewards Program Overview

• All eligible activities must be completed, and rewards redeemed in 2026.
• Members will receive a letter in the mail listing all 2026 eligible activities. Members will only be rewarded once for each eligible activity completed.
• Submit your Jefferson Health Plans Medicare Advantage members' claims correctly and in a timely manner so they can be rewarded for their completed activities.

• If your patients have questions about Wellness Rewards, they should call Member Relations at 1-866-901-8000 (TTY 1-877-454-8477).
