If your practice meets the criteria outlined below, please ensure the required training is completed.

Practices with at least one Jefferson Health Plans Medicare Advantage Dual Special Needs Plan (D‑SNP) member assigned are required to have at least one staff member involved in the care of D‑SNP members to complete the annual D‑SNP Model of Care (MOC) training. This training is required to remain compliant with Centers for Medicare & Medicaid Services (CMS) guidelines.

COLLABORATIVE CARE TEAM You play a vital role in supporting our D-SNP members. This training ensures all care team members understand the program and their responsibilities. 

COMPLETING THE ANNUAL D-SNP MODEL OF CARE TRAINING To maintain compliance with CMS requirements, you can complete this course at hpplans.com/provider-online-courses within 30 days of receiving this letter. Completing online should not take more than 15 minutes. 

When you enter your tax ID in the attestation at the end of the online training, it will display all associated sites. Be sure to check off each site that you are attesting for or select all sites before submitting your training attestation.
