We are committed to ensuring that all our members have access to high-quality care in an environment that is welcoming and accessible to everyone. As a participating provider, we appreciate your partnership and dedication to serving our diverse member population.
This is a reminder that, as part of our ongoing efforts to meet federal and state regulatory requirements, all participating providers are required to complete the Americans with Disabilities Act (ADA) Compliance Attestation. Completing this attestation is essential in confirming that your practice locations meet ADA standards and that our members—regardless of disability—can receive equitable access to care.
What does this mean for your practice?
- Your facility must be accessible to individuals with disabilities, including (but not limited to) accessible entrances, restrooms, parking, and examination areas.
- Your practice must provide reasonable accommodations for individuals with disabilities as required by law.
- Your staff must be aware of and trained on ADA requirements to ensure equitable service for all members.
Action Required:
Please complete the ADA Compliance Attestation Form.
If you have any questions or require assistance regarding ADA compliance or the attestation process, please send an email to providereducation@jeffersonhealthplans.com.
