New and current members can learn more about how Jefferson Health Plans is supporting a smooth and safe transition each contract year for their Medicare Part D prescription benefit.

About Our Part D Transition Approach

Current and new members, upon enrollment, will receive important prescription drug benefit information including:

  • A list of available medications
  • Plan requirements and coverage limits
  • The process for requesting prior authorization or exceptions
  • How to use our online directory to locate network pharmacies, and how to request a printed directory

There may be times when you are taking a drug not covered by Jefferson Health Plans, or our formulary changes and a drug is no longer covered or is subject to new utilization management requirements. This requires collaboration between your care team and Jefferson Health Plans to ensure you’re getting the medication you need.
