June 23, 2026 - A systems error recently caused some member communications to be sent to unintended Jefferson Health Plans (formerly Health Partners Plans) Medicaid and CHIP recipients. The issue was identified on April 24, 2026.

In response to the incident, Jefferson Health Plans took immediate action to correct the situation and safeguard the privacy and security of its members’ personal information. New ID cards have been mailed to the correct addresses, and measures have been put in place to prevent similar future situations.

To ensure affected members are informed, Jefferson Health Plans is reaching out to individuals directly through a phone call and a notification mailing about the incident. Through these communications, individuals are advised to send back or destroy any incorrectly received cards or materials.
