Special Supplemental Benefits for the Chronically Ill (SSBCI)

Jefferson Health Plans offers Special Supplemental Benefits for the Chronically Ill (SSBCI) for members in our Special, Dual Pearl, and Select plans. These benefits allow members to use the Flex Card to buy healthy foods and pay utility bills.

Who can receive SSBCI?

These benefits are available to members with at least one of the health conditions listed below. Not all members will qualify. If you are a member and have questions about these conditions, please call Member Relations at 1-866-901-8000 (TTY 1-877-454-8477).

  • Cancer
  • Cardiovascular Conditions
  • Chronic Alcohol Use & Substance Use Disorders
  • Chronic & Disabling Mental Health Conditions
  • Chronic GI Disease
  • Chronic Heart Failure
  • Chronic Kidney Disease
  • Chronic Lung Disorders
  • Cognitive Impairment
  • Dementia
  • Diabetes Mellitus
  • Functional Challenges
  • HIV/AIDS
  • Immunodeficiency & Immunosuppressive Disorders
  • Neurologic Disorders
  • Overweight & Obesity
  • Post-Organ Transplantation
  • Severe Hematologic Disorders
  • Stroke

Eligibility

These benefits are part of a special supplemental program for the chronically ill as defined by CMS. Members should contact Member Services for a detailed list of qualifying conditions.

  • If you have an associated claim on record with Jefferson Health Plans, you do not need to take any action and will automatically have access to these benefits.
  • If you complete a Health Assessment with Jefferson Health Plans and indicated a qualifying condition, you will have access to the benefit within 72 hours.
  • If there is no claim or Health Assessment on record, you may self-report a qualifying condition to access the benefit. To self-report, please call 1-877-205-8005 (TTY 711). Once reported, you will have access to the benefit within 72 hours for a total of 30 days.
