A systems error recently caused some member communications to be sent to unintended Jefferson Health Plans (formerly Health Partners Plans) Medicaid and CHIP recipients. The issue was identified on April 24, 2026.
In response to the incident, Jefferson Health Plans took immediate action to correct the situation and safeguard the privacy and security of its members’ personal information. New ID cards have been mailed to the correct addresses, and measures have been put in place to prevent similar future situations.
To ensure affected members are informed, Jefferson Health Plans is reaching out to individuals directly through a phone call and a notification mailing about the incident. Through these communications, individuals are advised to send back or destroy any incorrectly received cards or materials.
For further assistance or information, members can contact Jefferson Health Plans Member Relations at 1-800-553-0784 (TTY 1-877-454-8477) for Medicaid and 1-888-888-1211 (TTY 1-877-454-8477) for CHIP.
If you receive both Medicaid and Medicare and live in the state of Pennsylvania, you need to take steps to verify your information with the state to ensure you continue to receive health care coverage.
Verify Your Information
Once the COVID-19 Public Health Emergency ended, state agencies began evaluating each person’s Medicaid eligibility. People with Medicaid benefits could lose their coverage if they don’t provide the information necessary to verify their income or residency.
If you are a member of Jefferson Health Plans Medicare and also receive Medicaid benefits, you should take the following steps to make sure you receive your Medicaid renewal application paperwork.
You could lose coverage if you do not receive and complete your renewal form.
