Diabetes Prevention Program

Have you been told you’re at risk of Type 2 diabetes? If you have, help is available, and Jefferson Health Plans EverWell gives you access to the Diabetes Prevention Program.

Health Education Advisory Committee (HEAC)

Jefferson Health Plans EverWell has a Health Education Advisory Committee (HEAC) that is comprised of our Medicaid members, employees and community members. This committee meets to share ideas about our programs and services and let us know how we can do things better. 

HEAC meets four times per year. Members of the committee may be eligible to receive $50 for every meeting they attend.

Dental Exception Form

If you need a dental service that is medically necessary but is not covered by your dental plan or goes past your plan's usual limits, please work with your dentist to complete the form linked below to start a benefit limit exception (BLE) request.

Third-Party Liability (TPL)

As a Medicaid member, you may also have Medicare or other health insurance. This other insurance is known as Third-Party Liability (TPL) and is considered your primary insurance.

Health Survey
