An enhanced PPO plan that includes prescription drug coverage. Premier provides core medical benefits and additional benefits, including:

  • $55 montly plan premium
  • $0 copay for primary care visits
  • $20 copay for specialist visits
  • $10 copay for urgent care visits
  • $550 Flex Card for dental, vision and hearing spending
  • $200 eyewear allowance
  • Coverage for hearing aids
  • SilverSneakers® fitness membership
  • JeffConnect® virtual care
